No, don't select "yes" if you didn't pay medical expenses... there is no deduction for billed/unpaid expenses. You must actually pay them to be able to deduct them (with cash, check credit card etc... ).

No.  You cannot claim unpaid medical expenses or medical expenses that were paid for by insurance.
