What do I put down for "where I did business at" if I just incorporated my business in Dec 2018 and haven't done any business yet nor foreign qualified?
Do I have to list a state where I did business at when I just started my business at the end of December and haven't done any business yet nor foreign qualified?

For your purposes, put where you intend to be doing business. That'll work for IRS purposes, and will be consistent with you next return.
