Should I select one sale at a time or summary for 1099-B?

I have three 1099-Bs for one mutual fund sale.  One with Box A, one with Box D, one with Box B.  I chose to "enter summary for each sales category", but turbo tax treats each category like a new sale.  Do I just select "enter one sale at a time" and treat each 1099-B as a separate sale?

Yes, you should select "enter one sale at a time" and treat each Form 1099-B as a separate sale.  In your case, you have one sale split into different categories, not multiple sales per category.
