printing final return - missing form b on an itemized return

when I tried to print out my return to file by mail - after review found that i am missing schedule B.

I have 1099 div and 1099 int income, and I have an itemized return. I know this form is required since a couple of year ago I did forget to file it and IRS contacted me to that fact.

my question is why is this form not being included in the final print out, and mor important what other forms have been reported missing?

You only get Schedule B if either your interest or dividends is more than $1,500 (not combined). Do you have more?
