My W-2 shows my married name & new address, while my HSA 1099-SA shows my maiden name & old address. I called HSA; no help. Do I need to do something? Can I file as is?

The name and addresses on those forms do not matter as long as your Social Security number is correct and you have entered your name and SS number on your tax return exactly as it is shown on your current Social Security card.
