Module asks "What type of high deductible health plan did my wife have on DEC. 1, 2023. She was on my family HDHP but then it states there was a lapse.

We have had coverage for 15+ years with no lapse. The next screen asks why there was a lapse. So, was the original question pertaining to her own employer's HDHP, which she never opted into? I thought the family designation would have shown her covered by my HDHP, which is why I selected it.

Yes.

Take a closer look at the question on the screen just prior to the one telling you there was a lapse in coverage.

If both you and your spouse are covered under the same Family plan, then the person with the HDHP in their name will indicate they had a family plan.  The other person will indicate they had None because the HDHP was not in their name.
