It's asking "Was this loan paid off or refinanced with a different lender in 2016?" would the answer be "yes" if I refinanced with the same lender in 2016?

Yes, if your mortgage loan was paid off or refinanced, answer "yes" when asked 

"Was this loan paid off or refinanced with a different lender in 2016?" 

(even if you refinanced with the same lender).
