Im looking at my 1099-SA and i dont see anything for payer name. the account is held at fidelity, is that the info i give?

Yes, the "payer" is the institution administering your HSA, presumable Fidelity. Is there a "federal identification number" on the 1099-SA? That needs to be there as well.

If these items are missing, you might call Fidelity and ask them for the data that should be there.
