IF I TOOK THE STANDARD DEDUCTIONS? DO I STILL ADD MY $480 CREDIT DATA

WHERE DO I ADD THE 1040A OR 1040EZ

If the 480 was a state tax refund, no you don't add that as income. 

That would be on Form 1099-G

If you took the Standard Deduction on LAST year's federal return, do not enter the 1099-G this year.
