If you didnt fill out an w9 when you started referee you would entef it under other income.

You are considered self-employed with a side business offering referee services.  From your onw records report the income of your business.

You can deduct legitimate expenses such as mileage to out of town games, any clinics or certification fees, uniform you purchased last year.  My sons worked as soccer referees for several years.
