I read on one of the FEMA sites that when a disaster has been designated and a mandatory evacuation for the area you live in has been announced, you can count the hotel stay and cost of the evacuation, including some meals. Which form is used?

You can make a claim to FEMA for reimbursements for those expenses.  There is not a tax deduction for those expenses.
