I have unusally large medical bills. Should I include a description of why with my paper return?

No, most returns are processed with little human interaction and no one might read the reason why.

Most audits are selected by the computer and the reason listed would have no impact in the that case.

Just have all the documentation ready should IRS ask to see it at a later date.
