I have an HSA account but I did not make any contributions only my employer did so do I need to file a form for it?

If your employer made the contributions that should be noted on your W-2 by an amount in Box 12 coded "W".  So all you need to do is enter your W-2, if you also did not take any money out of the HSA.  If you took money out then you should get a 1099-SA and you will need to enter that.
