I have a UHC rewards program which adds funds to my HSA periodically for staying active. Should I report this as contributions made on my behalf in line 2 of form 8889?

These rewards funds are not listed anywhere else on my tax documents and are not included with employer contributions on my W-2.

Yes, you are required to report all contributions to your HSA, from any source.

 

These contributions should be reported as "personal" contributions in the HSA interview in TurboTax on the screen headed "Let's enter [name]'s HSA contributions (see screenshot). The total personal contributions will appear on line 12 of Schedule 1 (Form 1040) and will also appear on line 2 of form 8889.
