I have a form R-10614. I am trying to add it to my child care provider list but do not see a place to add.

To enter information for the School Readiness Tax Credit:

1. Go back into your LA return by clicking State in the black bar on the left of your screen.
2. On the Revisit Your Louisiana Info screen, click on Update next to Taxes and Credits.  
3. Continue to the screen, Louisiana Refundable Priority 2 Credits.  
4. Click the down arrow on the Select Description box and select the credit from the list.
5. Enter the credit amount and click Done when finished.
