I check my HSA statements from 2022, I contributed 240 and my employer contributed 500. But in box 12 of my W2, the employer contribution is shown as 740. Is this wrong?

It's weird that it lumps my contribution and my employer's contributions BOTH into "employer contributions", but I'm not sure if this is an actual problem.

Your form W-2 is correct. 

 

Box 12 code W includes both employee and employer contributions to your HSA. They all count in the the allowable contribution.

do not duplicate that $240 by entering it as your contributions on the 8889.  your wages (box 1 of w02)  were reduced by that amount so that's how you get the deduction entering it as your own would be double-dipping
