How do I enter Canadian form T5008?

I live in the US but my company is based in Canada.  I sold some ISOs last year that I exercised the year before. They sent me a T5008 but not a 1099-B.  How do I enter this information?

When you go to report a sale of stock on TurboTax it will ask if you received a 1099-B.  Tell it no.  Then it will ask for the information about the stock you sold.  Take the information from the T5008 and enter it there.  All set.

Here is the info on a T5008.

Turbo tax did not ask me if I had a 1099-b. It just started asking for info. I did:

add investments

Continue

enter a different way

Stocks bonds mutual funds

rhen it starts asking for info from my 1099-b
