How do I answer the ?, What type of HDHP did spouse have on 12/01/2020. His coverage was through my employer?

Was your spouse covered under their own HDHP plan?  If so, was the plan Self-only or Family?

 

Was your spouse covered under your HDHP plan which has Family coverage? If that is the case, you would answer None.

 

On your IRS Form 8889 Part I line 1, are you reporting Self-only or Family?
