How can I apply my Home Heating Credit information I received from my Consumers Energy bill?

Does the MI-1040CR-7 form need to be submitted separately?

The form MI-1040CR-7 is used for your home heating credit. It can be e-filed along with your state or it can be mailed in separately. Check your status at Individual Income Tax eService.
