Form 8862 - Both 'Yes' and 'No' checked in Part III?? A bug?

I am trying to fill out Part III of Form 8862, but TurboTax appears not to reflect my selection in the Worksheet.

I have two children, one is identified in question 12 as "Child 1". My second kid is identified in question 13 as "Other dependent 1".

No matter my selections, TurboTax seems to ALWAYS check 'No' for "Other dependent 1/2/3/4' for both questions 16 and 17. By default, they should all be BLANK. Selecting 'Yes' in the Worksheet does indeed check 'Yes' on the form, but it does NOT get rid of the check in 'No', so I get:

Question 15: Other dependent 1   xYes  xNo        instead of just:   xYes    _No

Question 16: Other dependent 1   xYes  xNo        instead of just:   xYes    _No

I am unable to deselect the errant 'No's.

Please advise. Thanks!

FIXED: Found the problem-- the Form 8862 template used by TurboTax has the No boxes checked. (The template should have nothing checked.)
HOW: I took the us_ind_8862.pdf and edited it using a PDF editor app, wiped out the 'x' in the No checkboxes and saved.
Note: Replacing the TurboTax us_ind_8862.pdf  with the official, downloaded IRS PDF did not worked.
