For the medical questions section, do I include only what I paid out of pocket or what I was billed, please advise. Thanks!

For medical expenses, only what you paid with out of pocket funds in 2016 can be entered as an itemized deduction on Schedule A.  If you were reimbursed for any medical expenses, those expenses are not entered on a tax return.
