For Medical Care deductions. Do you have to actually pay the medical bills to be able to claim them? I have $22,000 medical debt incurred this year.

Yes you must have paid with out of pocket funds medical expenses during the year to deduct the expenses paid.  Having just a bill is not paying for the expense.
