Does the 1099G come from the Unemployment Office or from your employer?

Form 1099-G to report unemployment payments would be  issued by your state unemployment commission, not your employer. 

These forms are required to be mailed or available to you by January 31st.  If your state unemployment commission has an electronic reporting system where you log in, the form might be available for download there.
