Do i need to include fsa/flex plan on filing my taxes

No, There are no reporting requirements for a FSA (Flexible Spending Account) on your tax return. 

FSA contributions are funded through a salary reduction (pre-tax dollars) by your employer. Because of the salary reduction, there are no taxes paid on the amounts. 

Note: If you paid for qualified expenses through your FSA (pre-tax dollars), do not deduct those expenses on schedule A, if you itemize. See below link.
