Where on the W2 is the insurance/benefit surcharge? is it the IRC125?

[Edited] Yes, the insurance/benefits surcharge on your W2 is the IRC125.  It is located in Box 14 of your W2.

I entered the amount from my W-2, box 14, code IRC 125, into the field for this question in the New York interview for insurance and benefits surcharge. Now I see that on New York Form IT-225, that IRC 125 amount is listed _twice_ as an addition, under codes A-101 and A-112. That doesn't seem right. Instructions for Form IT-225 are at: https://www.tax.ny.gov/pdf/current_forms/it/it225i.pdf.
A-101 is: "New York City flexible benefits program (IRC 125)"
A-112 is: "Health insurance and the welfare benefit fund surcharge"
Looks like I'll have to file an amended return :( .

Update: I contacted the NYC Dept. of Finance. Their emailed reply, and I quote:
"As a NYC Department of Education employee you are not subject section 1127 filings.
For further information on requesting a refund or more information on 1127 please call [TT does not allow phone numbers in comments]." (two one two - two nine one - two eight eight one; I did not find this listed on the web)

When you enter a NY W-2 with this code and amount in Box 14, the program comes to a screen following the W-2 entry that asks if "this amount was deducted or deferred from your salary under a flexible benefits program established by NY City or other NYC public employer on your behalf".  You can answer Yes or No..  Your answer here determines whether the amount becomes taxable or not on the NY return.  

If you said Yes, the amount was already on your NY return and you duplicated it by entering the amount again in relation to another question in the NY program.

What if my box 14 has two line items?

Then you report both line items. Below Box 14 is an entry that says + add another row, which gives you an opportunity to add another entry.
