Where is the 1099-R option for the US Armed Forces Retirees

Where is the 1099-R option for the US Armed Forces Retirees.  I can see civil services, railroad, 401K but not the one for the 1099-R  U.S. Armed Forces Retirees

It is the standard Form 1099-R on the page - Financial institution or other provider (1099-R)
