When completing a case information sheet for a family court matter, it asks for total income tax including Federal, state and local tax. Is Medicare tax included?

That's not a question about tax law. You have to ask the court, or your lawyer, exactly what to include. My guess would be that Medicare tax would not be included, but again, ask your lawyer or the court to clarify the question.
