RE: THE OREGON VSI(TP) FORM, I'M CHOOSING NOT TO FILL IT OUT, BEFORE I DELETE THE FORM IS THERE ANY ADVANTAGE TO FILLING THAT OUT ?

No, you can delete it.  The form is voluntary, and you aren't required to include it.
