PA-8453 Form: Signature Requirements

Turbotax advised me to sign PA-8453 form and retain it in my records.

 

This form has entries for 3 signatures: The taxpayer, the ERO, and the paid preparer.

 

Do I just sign and date the taxpayer area?

 

If this form is requested by the state, do I send it to them with the ERO and paid preparer section blank?

Yes, you can sign the form and just keep it with your tax records for three years.  It does not need to be sent in to the Pennsylvania Department of Revenue.

 

If PA requires additional information in the future, they will contact you to request it.
