My wife and I are senior citizen PA residents with less than $125 in Ohio income (from mineral rights royalties). Is this below the threshold requiring an Ohio tax form?

Yes.  $125 is below  all the Ohio exemption amounts.  Therefore you do not have to file an Ohio tax return.  But you must include that income on your PA tax return.
