Is there a place to add income from a slip-and-fall lawsuit? Fall was in MN and I live in TX. The attorney didn't provide any documents for taxes.

I was involved in a Slip and Fall accident and received a settlement. I am trying to figure out if/where to put it on my tax return.

If the settlement was for medical expenses and pain & suffering then the settlement payment is not taxable and doesn't need to be reported.  If it was for pain and suffering, then that part is taxable as "other income".
