If I was in FL for 4 mths out of the year from June - October, do I need to say that i moved there?

I did not work those 4 months in florida but i did  changed my address so i could get my mail throughout those 4 months as well as had health insurance so i didn't go without for 4 months.

No, you do not need to say that you moved to Florida for 4 months considering you did not work in Florida.

HI! i appreciate your answer. Even though i changed my address for everything and (somewhat) had Health insurance there as well? my home is in MA but i went to FL and came back home. i dont have to say anything about Florida?

That is correct.  You do not have mention Florida as you did not obtain any sources of income from Florida and are not (were not) deemed a resident of Florida.
