How do I find the certificate number for the IT-540 form?

So I’m new to filing taxes this is the first year I’ll be doing it. After completing all the forms I was prompted to give a certificate number for a IT-540 form however I do not have that form.

This appears to be a Louisiana state tax issue.  There are several credits available for LA that require a certification from another entity to qualify for the credit.  Is there a description/name given for the certificate?
