How do I fill in IT-540 Explanation of Changes

So I am doing my state amendment (Louisiana) and Turbo Tax asked me the reason, I put "American Rescue Plan Unemployment"... This gives me a IT-540 Explanation of changes form which has a grid of 4 columns, Explanation, Original, Change, Correct.

I have one row with my Explanation in the Explanation, Original/Change/Correct are blank....

 

Do I need to put anything in these cells? In the other rows? I've googled online for this form and can't find it any guidance on it at all, it's like it shouldn't exist.....

No, the explanation should be sufficient. Per the State of Louisiana, a corrected return for the tax year being amended should be submitted along with an explanation of the change(s). The amended return should look as if the original return was not filed, which will be automatically done with TurboTax. 

 

How can I file an amended return?
