Family Leave reporting

I had a family leave last year and I am confused when it comes to the state taxes that is asking me to enter 
PFL - Income Received from Insurance Company, where do I find this information?

Also I received a W-2 from SCMS Administrative Service Inc. and I believe this is related to the payments I received from the insurance company for my family leave, is this correct?  If this is the case, I didn't include it in my federal because the form arrive after I submitted my federal taxes; how do I proceed to adjust my federal state taxes for this W-2?

If you received personal family leave through an insurance company, then the amount would not be on your W-2, so the amount is probably the W-2 from SCMS Administrative Service.

 

If you filed your return already and it was accepted by the IRS, file an amended return.

 

The amendment form, Form 1040-X, is not ready in TurboTax. It’s expected to be ready around Feb. 23.

 

If you originally filed your return with TurboTax, we can help streamline the steps outlined above. 

 

If you used TurboTax Online, simply log in to your account and select “Amend a return that was filed and accepted.” 

 

If you used our CD/download product, sign back into your return and select “Amend a filed return.”
