"Check this Entry" is asking for a date, but just asks over and over and over. Is the form broken?

It is asking for a Date (from my stock portfolio), but when I enter the date, formatted correctly mm/dd/yyyy and I hit continue, it just asks me again.  I've musta answered this same question a few dozen times now.  I get the feeling this form is broken and I cannot answer this correctly.  What can I do.  it will not let me e-file until this is "fixed", but I cannot fix it.

You may want to try clearing your cache and deleting cookies and see if the issue persists. If it does, then you may need to delete the form and then re-enter the information; those steps can be found here.
