Are you required to fill in an amount for death benefit exclusion. What amounts should I list for death benefit exclusion? Any help is much appreciated.

employer-paid death benefit?

amounts up to $5,000 which are paid to the beneficiaries or the estate of an employee, or former employee, by or on behalf of an employer and by reason of the death of the employee shall be excluded from the gross income of the recipient.

 

this is not reported on the recipient taxpayer's return.
