Why when I filled out the 1099-R form, did it not ask where I rolled my pension over to?

Thought the IRS would want that info to confirm rollover?

What was the code in box 7 of the 1099-R?  Was the box for IRA/SEP/SIMPLE checked?  Was there an amount in box 2a of the form?

Box 7 Code G        Box not checked    No amt. in box 2a

A code G is automatically a rollover to a Traditional IRA unless you answer YES to the two questions about rolling into a Roth 401(k) or Roth IRA, if neither, then the default is a Traditional IRA so there is no need to ask any more questions.

Code G already indicates the amount distributed was rolled over -

G—Direct rollover of a distribution to a qualified plan, a section 403(b) plan, a governmental section 457(b) plan, or an IRA.

 

Rollover is entered on your Form 1040 Line 4
