Why did turbo tax not ask for a 5498 form to confirm where my pension rollover was rolled over to?

It did ask me to fill out manually a 1099-R which had code G in box 7  No amt in box 2a and box ira/sep/simple not checked

If you rolled over your Pension to an IRA, the IRA/SEP/Simple box should not have been checked.  A pension plan is not one of these.  Box 2a shows the portion of the distribution which is generally taxable. If there is no entry in this box, the taxable amount was not determined by the payer and it will have to be determined by the taxpayer.  Code G in Box 7 indicates Direct rollover of a distribution to a qualified plan, a section 403(b) plan, a governmental section 457(b) plan, or an IRA.  Distributions with a code G are not taxable.  You shouldn't need a Form 5498 to report your rollover.

The IRS does not need to know where you rolled your money to.  The code G in box 7 confirms it is a rollover.  A rollover is not taxable so that is the reason there is no amount in box 2a.  Evidently the account you rolled over was not an IRA/SEP/Simple.  Was it maybe a 401k account?  Form 5498 is not required to be included with your return.

They made check out to Fidelity as TTEE with FBO my name IRA, sent it to me and I mailed it to Fidelity.  Does explain why the box not check and still no other forms needed?

It normally would not require a 5498 to confirm where your pension was rolled over to. Sometimes it asks for a 5498 to report any nondeductible contributions that may need to be reported on a 8606.
