Who is the payer on form 1099-R? Would it be the financial org. that manages my retirement acount?

Typically the payer is the name of the Brokerage Firm, Bank, etc where your retirement funds are located or managed.

What if the Payer's (bank) address is in MN and the state Payer's State No. is WI?  Turbo Tax won't let me have two different zip codes.
