Which section or code do you use from W-2 box 12 for "contributions you made personally (not via employer)"? I have Codes C, D, W, AA, and DD with values.

If you're referring to HSA contributions, all of your contributions that are made through payroll deduction are considered as made via your employer, code W. Contributions made personally would not be shown on the W-2.
