I'm confused about the 403B plan. I have received a 1099-R form. My former employer paid the money into the 403B account. I never made any contributions. Where on here do I file this as my W-2 from my former employer states a retirement plan?

If your employer made contributions then this would be filed as your W-2 even if the 403B retirement. All retirement income that was contributed will be entered as part of your W-2.
