Where on the 1099R form can i find the Wells Fargo Advisors Username, and the Wells Fargo Advisors Password????? PLEASE

It is not on the 1099-R.   Rather, the program is asking for the User ID and password that you set up online for your Wells Fargo account.  Since you have the paper 1099-R, you can just manually type into Federal > Wages and Income > Retirement.  Scroll down to the bottom of that screen, and click on I'll Enter it Myself.
