When asked for the total in box 12 on my W-2’s, do I add up each one ie 12a-d

Where are you asked for a total for box 12 on your W-2? These items are totally different from each other and normally would not be added up.

Or can you read the question as something like "Enter the total health insurance premiums (code DD) from box 12 on your W-2"?
