What happens if I don't have information on lines 3,4,and 5 of a 1099-B but I know the proceeds?

Those lines on a 1099-B are not for proceeds.

Nonetheless, your tax document needs to contain all of the information that you need.  If there is information missing, you should contact the issuer of the 1099-B and request a corrected form.
