What forms and documentation are required to file GA form 500? I have 5 pages of form 500 and 1099R. Am I missing anything?

Retired. So only 1099R has my income. There were 5 pages of GA 500 form. Anything else I need to submit?

You would need all pages 1-5 of Form 500 for processing and you should have a Schedule 1 Adjustments to Income Based on Georgia Law to show Subtraction from Income if you have a Retirement Income Exclusion.  GA Schedule 1 has 2 pages.  The second page calculates the maximum allowable retirement income exclusion.
