What do I do if I have Form 5498, but not 1099-R?

Turbotax says If you received a Form 5498, don't enter it in TurboTax. You'll find the information we need on your Form 1099-R. However, I only have 5498. What to do.

A Form 5498 is an informational form only and is not reported on a tax return.

 

Did you make a withdrawal or a rollover from a tax deferred retirement account in 2023?

Thank you. What I did is 'rollover' from a Plan IH9900 with voya to a rollover ira with fidelity 

(sorry, I'm quite new to all of this)

You will need to contact the Plan Administrator at Voya.  They should have sent you a Form 1099-R or they have the Form 1099-R available on their website for your account.
