Traditional IRA is the same as Roth IRA? I have 401k through work do I add that to either or none?

A 401k is a retirement account but it is not the same as an IRA.   When you enter your W-2 from your employer, the 401k information is already on your W-2, so you enter it from your W-2 only.  You do not enter it anywhere else.
