The IRA/SEP/SIMPLE box has an X in it, not a check and not blank, -do I consider the X as checked?

The IRA/SEP/SIMPLE box on my 1099R has an X in it. Do I consider the box checked?

Yes, "X" means the box is checked.  The distribution is related to an IRA / SEP / SIMPLE.
