State distribution amount (Box 16) in 1099-R for excess ROTH contribution?

I have removed excess ROTH IRA contributions made for tax year 2021 and I am reporting 1099-R for tax year 2021 based on the contributions + earnings (as per instructions in other threads on the same topic).
However, the instructions do not mention anything regarding the State distribution amount (box 16).

Should box 16 kept blank or should I use the amount in box 1 (Gross distribution amount) and mention it in box 16? My state is California and I did not withhold any Federal or State taxes upon removal of excess ROTH contribution. Thanks.

Yes, you will have to enter the Gross distribution amount from box 1 in box 16.

Thanks @DanaB27 for the quick response.

A quick follow-up query on the same - can I leave box 15 blank (State ID/ Payer's State No)? I don't have these details with me as I don't have the 1099-R yet (which would likely be mailed only next year). I prefer to not have to amend the tax return if the actual 1099-R (that would be mailed next year) ends up having different information from what I put in my tax return, hence being cautious. Thanks!

You can check with your financial institute to see if they can give you the information for box 15.
